program is desperately needed. The insurance marketplace currently offers optical benefit plans that cost an average of $15 per
employee per month. This capitation system requires payment for the entire employee base, regardless of usage.
Consumers face deductibles and surcharges for anything more than the most basic eyeglasses.
Opti-Vision Benefit of America (OVBA) was developed as an affordable alternative for vision and optical care. This program is offered
through our Exclusive Provider Organization (EPO).
With OVBA the employer is charged only when the employee utilizes the program. Statistics
show that only 1/5 of the population will utilize optical benefits resulting in 75% savings to the employer.
The OVBA vision plan allows the employer to offer an attractive, low cost vision plan
that is unique in the market today.

Our annual standard benefit plan of $149 for single vision glasses or $159 for bifocals entitles the employee and/or family member to:
- One comprehensive eye exam
- Eyeglass Frames: one pair of standard frame.
- Choice of the following lenses- Single Vision, Flat Top 25, Flat Top 28, Round 22.
- Soft contact lenses offered at the 20 % discount off the usual and customary fee includes an eye exam, all fittings and any required training.
- A large selection of eyewear at attractive prices.
- Employer is charged only for glasses purchased by his employees.
- A network of eye care providers is located in your neighborhood.

| FOUR OPTIONS FOR THE EMPLOYER |
| Plan 1 |
NO COST TO EMPLOYER
The employee can join the program as an independent member or in an organized group. The employee pays all of the expenses, including the
administrative fee when services and optical goods are rendered. The employee is required to register via the internet - www.OVBA.net - or by calling 800-809-6822.

Key Benefits for Employee
- There is no cost to the employer.
- Employees join as an independent organized group.
- Employees are charged an administrative fee of $12.50 for individuals and $18.75
for families
(a family consists of 1 or 2 parents and up to 3 children.) If the family has more members, an administrative fee of $35.00
will be charged annually. The administrative fee is charged only when benefits are utilized. If benefits are not utilized,
the administrative fee will not be imposed for that particular year and membership remains intact.
- The Basic Package includes a comprehensive eye exam, 2 pairs of glasses and contact lenses.
- 1st pair of single vision glasses costs $149.00 or $159.00 for bifocal glasses.
( Eye exam, frame & Cr-39 lenses)
- 2nd pair of single vision glasses costs $89.00 or $99.00 for a 2nd pair of bifocal glasses.(frame & Cr-39 lenses)
- Save $60. If your current medical insurance covers an eye exam, you pay only $89.00 for the first pair of single vision glasses and $99.00 for bifocal glasses.
- One pair of professionally fitted contact lenses, a comprehensive eye exam and any required training is available at 20% discount off the usual and the customary provider's fee.
Hard, gas permeable contact lenses are available at an additional low fee.
- Items may be purchased separately: Eye Exam-$60.00,
Frame-$40.00, Single Vision Glasses-$49.00 and Bifocal Glasses- $59.00.
- No restriction on product selection. Designer frames and current technology lenses can be purchased for a nominal fee.

MINIMAL COST TO EMPLOYER
The employer pays only the minimal monthly administrative fee. The employee pays the cost of all services and goods.

LOW COST TO EMPLOYER
The employer pays a nominal monthly administrative fee and the cost of glasses purchased by his employees. Employer
pays for one eye exam and one pair of glasses or contact lenses per year. The employer is charged
only for employees who utilize the program.

- No monthly premium charge for each employee (no capitation system.)
Other plans require a monthly premium for each employee, but OVBA fees are paid ONLY when the
employee utilizes the program. (Statistics show that only 1/5 of the population will utilize this benefit resulting in
a savings of over 75%.)
- Employer's cost will be reduced by 85% when the employee pays the deductible.

- Employer pays only for the lenses, frames and a minimum administrative fee. This saves the
employer approximately 85% when the eye exam is paid under existing medical insurance.
- The cost to employer can be reduced significantly when the deductible is paid by the employee.
Deductible is paid when services are rendered.
- These plans enable the employer to offer additional coverage to their staff and to customize them to fit the needs of the employees.
